Annual Refiling Survey

Every three years, Ohio employers are asked to complete an Annual Refiling Survey via the Industry Verification Form (BLS-3023). Staff from the Ohio Department of Job and Family Services (ODJFS), in coordination with the U.S. Bureau of Labor Statistics (BLS), process collected data to verify and/or update information, including each employers’ industry, county, mailing and physical addresses, phone number, and multiple worksite status.

If you’re asked to complete this survey, please do! This report is mandatory under Ohio Unemployment Compensation Law, Section 4141.20 and is authorized by law, 29 U.S.C 2. Your cooperation is needed to make the results of this survey complete, accurate, and timely.

 

How to complete the report:
Reporting online is easiest.

  • Online:

    If you received an email prompt it was because you reported data for your company online in the past, or because your email is on file with Ohio’s Unemployment Insurance program.

    Log in to https://idcfars.bls.gov using the ID and password you were sent.

    If you didn’t receive login credentials, or if you didn’t get a form in the mail, you can download the Industry Verification Report - BLS 3023 and submit it by email, fax, or mail:
